The timer definitely moves between work and breaks, but if you're not seeing/hearing notifications you can easily miss a break. That will stop notifications for all apps from appearing. Another problem could be that you have 'Do Not Disturb' enabled. If you aren't seeing notifications or hearing a sound, please check the app's notification settings in the 'Notifications' section of Apple's System Preferences app and that the system alert sound is not set to zero volume in the 'Sound' section.
